How do I create a form?
This article explains how to create a form to capture additional information from customers or about products
Important: Since the release of version 2.4.13 in November 2024, the ability to build forms using the previous method has been discontinued. Any data that has been collected through these old forms will still be associated with the respective contacts - you can view and edit this data, but you are unable to make any changes to the forms themselves.
You can view your old forms in Organisation setup > Data collection > Old forms.
Before creating forms, you first need to create the fields, which act as the building blocks for forms. For help with fields, view our help article here.
Types of forms:
Customer forms
Used to gather additional information from customers when they make purchases via your shop, for example, for customers to pick additional options when purchasing merchandise.
Internal customer forms
To record more information about a contact โ these are visible to your organisation but not to contacts. For example, for coaches to add information about a childโs behaviour.
Product data forms
To store additional information against your products, for example, to label products with codes that align with your storage management or external applications.
Creating a form
- Navigate to Organisation setup > Data collection > Forms
- Click โAdd formโ in the top-right corner and choose what type
- Give your form a name
- Drag and drop your fields into the form from the left sidebar
- Click โAddโ below your fields for additional formatting tools (headers, descriptions, and dividers)
- You can reorder the contents by dragging and dropping
- Use the pencil icon to edit a field or the โXโ to remove a field or formatting tool
- You can preview your form, and when finished click โSaveโ in the top-right corner

Note: You can add both customer fields and internal customer fields to a form. Internal fields will be hidden from contacts.
