How do I link a form to a product?

Once you have created a customer or product data form, you can link the form to your products to start collecting data

From form settings:

  • Navigate to Organisation setup > Data collection > Forms
  • Click any form’s title
  • Click ‘Setup linked products’ in the top-right corner
  • Select the products/product groups to link the form to
  • Click ‘Save’

 

From product settings (customer forms only):

  • Navigate to Product management > Products
  • Select a product or product group from the left sidebar
  • Open ‘Forms’
  • Select the forms to link to the product

When your customers purchase any product linked to a form in the shop, they’ll be required to fill out the form during the checkout process.

Product data forms

To provide internal-only data for product forms, locate ‘Product fields’ in Product management > Products and click on the form.

Note: Internal customer forms cannot be linked to products - they can only be edited and viewed by admins/coaches of your organisation. More information available here.
